After Nido dell'Aquila beach at San Vincenzo (GPS: Lat. 43.031943, Long. 10.52584), other two beaches were officially designed as naturist: last year the Acquarilli beach, near Lacona village in the Elba Island (GPS: Lat. 42.763524 , Long. 10.334364), this year the small Buca rossa cove in Piombino (GPS: Lat. 42.920752 , Long. 10.541124).
If you consider that there is also the unofficial Marina di Bibbona beach (GPS: Lat. 43.205426 , Long. 10.534087), within 40 km you can find many naturist spots in one of most beautiful Italian coasts.

When we're in Tuscany my non-naturist family and I go to the beach at Follonica. Looks to me like the beach at Piombino can't be all that far from there. Are there many facilities there do you know, places for lunch etc. Thanks
There are only 25km from Follonica and Piombino.
Buca Rossa beach is a very small cove. You can park your car in Via Mina and reach the cove in few minutes walking on rocks. As it is in the heart of a small town, you can find hotels, restaurants, bars (not naturist) and so on, but you have to bring everithing you need at the beach as there is nothing there.
Otherwise, you could consider to go to Nido dell'Aquila beach at San Vincenzo (30 km from Follonica). The naturist beach is larger and there is a "mobile bar" there.

Thanks. Another question. Are either or both of these beaches clothing optional? By which I mean I think my wife is coming round to the idea of coming with me to a naturist beach, but she's a long way from being willing to strip off herself. Would we look out of place if I were naked and she in a costume?

Yes, all of them are free beaches and no one forces to be nude.
As I said, Buca Rossa is really a tiny cove so IMHO is better to preserve it only for naturists, but in Nido dell'Aquila she can feel comfortable as she preferes. It's crowded but quiet and many people started with naturism there.
